Care Philosophy
Veterans with heart failure and other cardiovascular diagnoses often have multiple, chronic illnesses, making the care of their heart disease very complex. My care of these patients is guided by several guiding principles, including: - Patient centered care and shared decision making in the complex care of older Veterans with heart failure and other cardiovascular diseases in the setting of multiple medical conditions and many prescription medications - Creating care plans that ensure Veterans can age in place as long as possible - Utilizing the VA's resources for remote monitoring of patients at home and administering care over the phone or by telehealth as much as possible to minimize Veterans' travel burden
